1. 公司简介(宣传册)，包括名称、地址、电话，传真号码，规模，业务，财务状况， 员工情况等。
Introduction (Company brochures), Name, permanent address, and phone and fax numbers, size, business, financial situation, employee situation of company.
2. 工作职位名称，职责叙述，工资及最低要求信息。
Job title of the position, along with a detailed description of duties, salary offered, and minimum qualifications for the job.
3. 代表公司签字的人名和头衔。
Name and title of company agent signing forms on employer’s behalf .
4. 公司的联邦税号，最新的报税表。
Federal tax ID number and the latest tax returns.
5. 公司信头纸（数张）。
Company letterhead (several copies) .
6. 申请费用，政府费用，反欺诈费用，劳动力培训费用。
Filling fee, Government fee, Anti-fraud fee, Labor training fee.
